Comparison grid of five cochlear implant systems and their 2026 status, for patients in Westchester

Cochlear implant news moves fast, and the headlines mix products you can receive today with products that are still in review. This guide separates them. It also explains what we do at Audiology Associates of Westchester, because the device is only one part of the decision.

Here is the plain status as of November 2, 2026. Cochlear Nucleus Nexa has FDA approval, with a decision dated July 3, 2025. Advanced Bionics and MED-EL sell current systems in the U.S. Fully implantable systems, such as MED-EL TICI and Envoy Acclaim, are not approved for sale in the U.S., and we give no date for either.

“Most of the outcome comes from timing, from the surgery and from programming and practice afterward.”

How we use this comparison

Dr. Caitlin Dold leads our cochlear implant work. We assess candidacy, explain the choices and, after surgery, provide activation and programming. Surgery takes place with an implant surgeon. We do not recommend a manufacturer before we know your hearing, your health and your goals, and we do not offer a device that lacks FDA approval.

Five systems side by side

The table lists status only. It gives no performance ranking, because we know of no independent head-to-head study that supports one. Dates come from the manufacturers or from public filings, and 'not stated' means we found no figure.

What matters more than the brand

Most of the outcome comes from timing, from the quality of the surgery and from programming and practice afterward. A candidacy test comes first. It combines a hearing test, a speech test in your best-aided condition and a medical review. If you wear hearing aids and still miss most words, ask for it. Our guide on when hearing aids are no longer enough explains the signs, and our severe and profound loss guidelines page shows the thresholds.

What the fully implantable systems would change

TICI from MED-EL and Acclaim from Envoy aim to place all parts under the skin. Envoy has told the FDA it is filing its application in parts. Its filing dates were June 30 and October 12, 2026, and the last part is due in the second quarter of 2027. MED-EL announced TICI UNICO on September 29, 2026, with a CE Mark for Europe. These are milestones, not approvals. If you are a candidate now, waiting for an unapproved device means living with the loss in the meantime, so we ask patients to decide on today's options.

If you already have a Nucleus implant

Nexa is a new processor and implant generation, per Cochlear. Whether an existing Nucleus user can move to Nexa depends on the implant you have. Ask Cochlear and your surgeon. Our team can read your records and help you ask the right questions.

Medical information notice

This page is for general education and is not medical advice. A cochlear implant is a surgical device, and candidacy depends on an individual evaluation. Sudden hearing loss needs urgent medical care. Call (914) 949-0034 to book.

Common questions

Is Cochlear Nucleus Nexa approved in the U.S.?

Yes. The FDA decision on the Nucleus Nexa system is dated July 3, 2025. Ask your surgeon which model fits your anatomy and hearing.

Which cochlear implant is best?

We know of no independent head-to-head study that names one best system. Fit, surgeon experience, your health and your goals decide. A candidacy visit is the first step.

Can I get a fully implantable cochlear implant now?

Not in the U.S. as of November 2, 2026. MED-EL TICI and Envoy Acclaim are not approved for sale here, and we give no date for either.

Do you perform cochlear implant surgery?

No. Our team handles candidacy, referral coordination, activation and programming with Dr. Caitlin Dold. Surgery is done by an implant surgeon.

Should I wait for a newer device?

Waiting means more time with limited hearing, and no date is set for the unapproved devices. Talk to the team about your timing, because delay has its own costs.

Can I upgrade my Nucleus processor to Nexa?

That depends on your current implant. Ask Cochlear and your surgeon, and bring your records to us if you want help reading them.

Get one email when an unapproved implant changes status.

We will send one email when the FDA acts on a fully implantable system or when a major status in this table changes.

We use your email only to send this update. No health information.
